A jazz lead sheet typically contains:

• melody
• chord symbols
• basic form

It usually does not specify exactly which voicing to play.

So if it says:

Dm7

you decide whether to play:

D-F-A-C

or:

F-C-E

or:

D-C-F

or another voicing.
